Abstract
30 year female, in second trimester pregnancy, presented with glaucoma with nanophthalmos and pseudophakia OU. OD BCVA 6/60, AL 17mm, 0.9 CDR, IOP 34mm Hg on GAT; OS BCVA PL, AL 17mm, 0.9 CDR, IOP 36mm Hg, on maximal topical and oral antiglaucoma therapy as per pregnancy, underwent OD trabeculectomy.
Surgery was performed with prior creation of scleral window, no antifibrotic agents used, slow decompression of anterior chamber, pre-placed scleral flap sutures, use of anterior chamber maintainer, tight scleral suturing with use of releasable sutures helped avoid intra-operative and early post-operative complications. The patient was atropinised postoperatively. Postop OD vision maintained, IOP 12mm, bleb well functional.
Conclusion- Management of eyes with nanophthalmos, glaucoma, one eyed status in pregnancy is a management challenge. Adequate precautions can help achieve favourable outcomes and avoid sight-threatening complications.
