Abstract
Methods: Medical history, clinical features, imaging and outcome were studied.
Result: A 45 year-old female with severe ptosis underwent frontalis sling surgery with implantation of silicone rod; presented with pain, swelling, photophobia and blurring of vision in left eye since last 2 months. Anterior segment examination revealed granuloma at surgery site and exposure keratopathy with ulcer measuring 2mm*1mm. Explantation of sling was done and patient was initiated on systemic antibiotics and fortified antibiotic eye drops. There was resolution of granuloma over a period of 7 days and corneal ulcer over a course of 3 weeks post explantation.
Conclusions: The granuloma formation after sling surgery occurs either primarily due to exposure of sling/sleeve enveloping the sling or suture tied around the sleeve. Resolution of these granulomas with antibiotics is usually incomplete but responds well to explantation. The patient could be considered for re-sling surgery after 3 months.
