Abstract
A 24 year old male presented to us with chief complaints of recurrent on and off swelling, redness, ptosis and diplopia since 6 months in left eye. Examination revealed severely restricted extraocular movements in downgaze in left eye with moderate ptosis, proptosis, and normal visual acuity .CT brain plus orbit done elsewhere was normal. Patient was thereafter advised MRI brain plus orbit which was suggestive of solitary cysticercosis cyst in superior rectus muscle. Patient was managed with a combination of oral albendazole and steroids. Patient responded quiet well to the treatment and the lesion resolved with treatment both clinically and on imaging. Cysticercosis has myriad clinical presentations depending on the site of lodgment. Isolated extraocular muscle involvement is rare and usually presents with nonspecific complaints. Many such cases are misclassified as pseudotumor and are initiated on steroid which gives temporary relief due to its anti-inflammatory effect.
