A 10 years old mentally challenged Down’s Syndrome boy was referred as a case of bilateral pan uveitis with total cataract in both eyes. He had cardiac diseases with pulmonary hypertension also. He had undergone extensive systemic evaluation for uveitis and was treated with systemic steroids in the past. On presentation he had total cataract in both eyes with hypotony. B scan was done which revealed a closed funnel retinal detachment in LE and inferior retinal detachment in RE. He underwent belt buckle with vitrectomy in the better right eye under high risk anesthesia. Intra operative findings revealed retinal detachment with giant retinal tear. This video highlights a meticulous step by step approach to this case that resulted in a successful outcome.
