VT0171 – EVACUATION OF A MASSIVE REBLEED UNDER SILICON OIL

Abstract
Our patient presented to us within a week With fundus obscuring rebleed under silicon oil after primary uncomplicated vitrectomy for TRD. Rebleed under oil presents as either fresh haemorrhagic patches or darkening in colour of the pre existing patches and oil is expected to confine the haemorrhage. Usually recurrent haemorrhages are not too massive. He was planned for immediate resurgery. After silicon oil removal, followed by clearing the fresh vitreous haemorrhage, we noticed a thick white sheet of old and altered blood adherent to the retina occupying about three quadrants. We started clearing the thick sheet of blood in layers by gently lifting it from the retinal surface and cutting with the cutter and from underneath red unaltered blood kept oozing out which was aspirated with vacuum. After what seemed like never ending haemorrhage, the vitreous cavity and retina were finally cleared off the blood. Fluid air exchange was done and repeat silicon oil tamponade was given.
