Abstract
Congenital anterior staphyloma (CAS) is a rare anomaly consisting of an enlarged, vascularized, ectatic cornea, with thinning and ectasia of the surrounding anterior segment structures. Management of CAS carries an extremely dismal prognosis with the inherent risk of phthisis, hypotony, and intraoperative expulsive hemorrhage. Surgical management includes keratoplasty, some resort to enucleation with prosthetic eye implant or DLCP as a globe salvaging. We hereby present a LE staphylectomy with lensectomy in a case of a 4-year-old girl with CAS. Her pre-operative visual acuity in RE was 6/6 (Cardiff) and LE PL+VE, and IOP was 8mmHg in both eyes. In the postoperative period, her vision was counting fingers and IOP of 8 mmHg. Moreover, she gained good cosmesis. A detailed examination under anesthesia with imaging modalities like ultrasound biomicroscopy and AS-OCT may give subtle clues in predicting success in such cases.
