Abstract

Synopsis- A premature infant with GA 31 wks and BW 1800 gms presented at 35 wks PCA after undergoing laser photocoagulation and receiving intravitreal ranibizumab injection in both eyes for AROP in zone1 elsewhere. Fundus examination showed a full thickness retinal break 1.5 disc diameter in size just temporal to macula with surrounding subretinal fluid extending upto fovea. Immediate laser retinopexy of this break did not succeed. Vitrectomy would have necessitated silicone oil injection inherent risk of complications such as glaucoma and cataract. A different approach using a radial sponge segment as a temporary explant to buckle the break was attempted. 3 weeks postoperatively retina attached. At 5 weeks postoperative the sponge was explanted with the retina remaining attached on subsequent follow ups with no recurrence of ROP. This video demonstrates a rare case of a rhegmatogenous retinal detachment with a posterior break in AROP and its unique surgical management.
