Abstract
Synopsis:55 year old female with history of failed VR surgery in LE-no PL and RE scleral buckle surgery 6years ago presented with RE sudden drop in vision since 1day.RE vision was HM+,pseudophakic with VH.B-scan showed VH+RD.Patient underwent PPV+SRFD+Endolaser+SOI.Due to persisting SRF patient was posted for re-surgery.Intraoperatively subretinal oil and buckle intrusion was noted.In view of 1eyed status and chances of loss of vision,buckle was left in situ and subretinal oil was removed with peripheral retinectomy,endolaser and silicon oil injection.Post-op vision was 5/60 with stable retina.
Highlihght:There are case reports describing removal of intruding buckle,however with risks of globe integrity loss.Management of each case should be tailored.It is not always necessary to remove the intruding buckle.It may be left intact unless there is significant threat to the integrity of ocular structure’s , recurrent RD or haemorrhage.
