Abstract

Performing phacoemulsification in a small pupil has always remained an enigma. A variety of instruments are designed and used in the form of pupil expanders or iris hooks. They all are indeed helpful and useful to complete a case by phacoemulsification.

I present before you the finer nuances, tricks, and techniques of doing phacoemulsification in the small pupil as small as 3.5-4 mm without using any accessories.

The surgeon developed the idea from the cases that develop intraoperative pupil miosis and were completed without using any adjuvant devices.

With the newer generation, phaco machines, and with enough experience in performing phaco one can successfully complete phaco without using iris hook or expander in a good percentage of the cases.

The aim is to build confidence in all experienced surgeons.
