Abstract

56 year old lady presented with left eye failed large macular hole ( > 1400 microns ), vision of CF @1 m , inverted ILM flap surgery was planned . 25 g vitrectomy was done , ILM was stained with brilliant blue dye, Macular massage was done to reduce the size of the hole and pedicle ILM graft was peeled and under PFCL moved over the hole. Fluid air exchange was initiated but ILM was dragged into the flute during exchange. Anterior lens capsule graft was dissected was placed over the macular hole, fluid air exchange was done followed by C3F8 gas injection. Prone positioning advised for 2 weeks post-operatively. The vision improved to CF @4M at 3 weeks with closure of macular hole . Anterior lens capsule can be used as a rescue graft for closure of large macular hole.
