Abstract
We aim to propose a uniform etiological classification and angiography based staging scheme for Lupus Retinopathy (LR) by retrospectively analyzing 21 patients (42 eyes) with Systemic Lupus Erythematosus (SLE). The etiological Classification was 1. Primary (P) LR- due to direct SLE related vascular/ neural involvement of retina or optic nerve; 2. Secondary LR- due to 2a-Hydroxychloroquine toxicity, 2b-hypertensive retinopathy (HR) secondary to SLE, 2c- blood dyscrasia related retinopathy (BRR); 3. Combined- P with HCQ retinopathy(3a), P with HR(3b), P with BRR(3c); 4. Associated with optic neuropathy; 5. No retinopathy. Angiography based staging was 1. Benign- having arteriolar attenuation only; 2. Occlusive- showing micro(2a) or macro(2b) occlusions; 3. Proliferative- with neovascularization at disc or elsewhere; 4. Complications- with vitreous hemorrhage or retinal detachment. This classification and staging system will help in determining a plausible management algorithm for LR.
