Abstract

AIM:A rare presentation of overhanging bleb post trabeculectomy with MMC.INTRODUCTION:Overhanging bleb is an oversized filtering bleb covering apart of cornea causing foreign body sensation,excessive tearing,vision changes.A post operative complication of trabeculectomy with MMC.

70y/M,h/o noticing a swelling in REx10days.H/o burning sensation,rubbing of eye.H/o RE-trab with MMC 2y ago.O/E:RE-BCVA6/12,IOP12mmHg,overhanging bleb-5x5mm,smooth surface,minimal vascularisation.PCIOL+Fundus0.7CDR,loss of inferior rim,macula-N.ASOCT-original bleb+,overhanging bleb+INTRAOPERATIVELY:Trypan blue dye was used to stain the overhanging bleb.it is ensured that dye didn’t enter in original bleb.Overhanging bleb bluntly separated from cornea,original bleb,then excised.3M postop:RE-BCVA6/6p,IOP11mmHg,bleb seidel test-ve.DISCUSSION:Topical lubrication,compression sutures can be helpful.Sutureless revision of overhanging blebs is safe,effective method for relieving symptoms and continued bleb function.
