Abstract
We describe two cases of Pigment epithelium detachment (PED) associated with age-related macular degeneration (AMD) with an unusual course. Case 1- 84-year-old gentleman, seen in 2018 with soft drusen and drusenoid PEDs, developed flattening of OS PED with development of Geographic atrophy (GA)over a course of 30 months. OD showed a RPE aperture at apex of drusenoid PED. The PED gradually progressed to GA in OS over next 6 months. Case 2- A 68-year-old-lady, presented in June 2018 with a large serous PED in OD, drusen in OS. The PED showed gradual increase in size over the next 13 months, with development of a spontaneous RPE rip. By April 2022, OD had atrophy at macula, with OS showing a large serous PED with increasing size. AMD can be associated with development of large PEDs- drusenoid or serous, in the absence of a neovascular membrane. Progressive changes in these, like an RPE aperture or a rip, can lead to geographic atrophy with ultimate poor visual outcomes.
