Abstract
This video demonstrates a modified surgical approach in a case of triple trouble( microcornea, iris coloboma with zonular weakness and hard mature cataract), for optimum visual outcome. Such cases are a challenge as phaco emulsification is often not possible due to weak zonules and the standard small incision cataract surgery can prove to be traumatic with high risk of descemets membrane detatchment. Extra capsular cataract extraction can be opted, but has risks of iris proplapse and wound leak. In this technique, a large sclero corneal tunnel is made to accommodate the dense nucleus and facilitate an atraumatic delivery. The tunnel is partly self sealing unlike the ECCE section. A three piece intra ocular lens was implanted in the patient and the vision improved from perception of light (PL) pre operatively to 6/18 post operatively.
