Abstract

PURPOSE: To describe an unexpected series of complications and their management in a logical way to achieve maximum possible outcome in a case of complicated suprahard cataract with 360 posterior synechiae.

METHODS: During routine phacoemulsification with a pupil retractor in a mid dilated complicated black cataract with 360 synechia, a significant amount of zonular laxity was detected.Capsular hooks were used to stabilize the bag complex.Before last fragment removal PC rent occurred.TA assisted anterior vitrectomy was done.After removal of capsular hooks accidental extension of rhexis margin noted.During delivery of multipiece IOL accidental reverse IOL unfolding occurred.After managing it there was an inadvertent entry of one haptic through the torn rhexis,while dialing the IOL. It was retrieved and ultimately placed in sulcus with help of end grasping forcep.

RESULT:Patient regained 6/9 BCVA after 1 week follow up. IOL was in center and stable in sulcus.
