Abstract

Surgical management of subluxated cataract is one of the major challenges . It can occur in marfans syndrome, pseudoexfoliation syndrome or trauma. Proper clinical evaluation to look for phacodonesis, degree of subluxation in clock hours, vitreous in anterior chamber, hard cataract is essential. Secondly surgical planning is an important step.

1. Incision: Make the main incision away from zonular weakness.

2. Capsulorrhexis : Completion of rhexis and bag preservation. Trypan blue dye must be stained just over the anterior capsule under visoelastic.

3. Stabilization of bag: Capsular hooks, CTS, CTR and Cionni rings to be placed. When to place a CTR?

4. Phacoemulsification : Repeated and meticulous hydrodissection must. low parameters is mandatory.

5. Longterm bag-fixation: Depending on the degree of zonular damage and its chance for progression either a CTR, CTR with Ahmed CTS or Cionni’s ring is used and sutured to sclera by 9/0 polypropylene ox Gortex sutures.
